Ready to Get Started? This form is for parents who would like to bring their child for an in-person trial lesson, as well as those enrolling their child in our class for a month or a semester. The fee for a trial lesson is $25.

How do I sign up for a trial lesson?

Fill out the form and email us at info@chesskiddo.com to schedule your child’s trial lesson. Trials are offered on Tuesdays, Wednesdays, and Saturdays. Please include your child’s availability (preferred days/times)

CHESSKIDDO PARTICIPATION AGREEMENT

In consideration of participation in Chesskiddo, I agree to indemnify and hold Chesskiddo harmless from any claims related to my child’s participation in its chess classes or events. I also permit emergency medical care for my child if necessary, with all medical expenses billed directly to me or my insurance.

MEDICAL AND LIABILITY AGREEMENT

I understand and accept Chesskiddo’s policies and the terms of this agreement, including the release of liability. Failure to comply will result in termination of membership. Full terms are available at chesskiddo.com.

PHOTO/VIDEO/SOCIAL MEDIA WAIVER

I allow Chesskiddo to use photographs, videos, and audio recordings of my child for promotional purposes unless I submit a Media Opt-Out form.

COMMUNICABLE DISEASE WAIVER

I acknowledge the risks of infectious diseases, including COVID-19, and agree to follow safety measures set by Chesskiddo. I release Chesskiddo from all liability related to illness, injury, or property loss.

ACKNOWLEDGEMENT

By signing below, I confirm I am the parent/legal guardian, have the authority to sign, and agree to all terms, including this waiver of liability. This prevents any claims or monetary recovery from Chesskiddo.
