Participation Agreement

Liability Waiver

I agree to indemnify and hold Chesskiddo harmless from any claims related to my child’s participation in its events. This includes releasing Chesskiddo from liability for any injuries, illnesses, or property loss incurred during participation.

Emergency Medical Care

I grant permission for Chesskiddo to authorize emergency medical care for my child if necessary. I understand that all medical expenses will be billed directly to me or my insurance provider.

Media Waiver

I allow Chesskiddo to use photographs, videos, and audio recordings of my child for promotional purposes unless I submit a Media Opt-Out form.

Communicable Disease Waiver

I acknowledge the risks of exposure to infectious diseases, including COVID-19, and agree to follow safety measures implemented by Chesskiddo. I release Chesskiddo from liability related to such illnesses.

Acknowledgement

By signing this agreement, I confirm that I am the parent or legal guardian, have the authority to sign, and agree to all terms. This prevents any claims or monetary recovery from Chesskiddo.
